Do they work exclusively in this niche?
A lot of agencies list "peptides" as one vertical among many. Ask how many active peptide clients they run right now, and whether peptide-specific compliance is something they specialize in or something they are learning on your account.

Do they understand platform policy before launch?
Meta and Google have real, specific rules around peptide and research-chemical language. Ask how they review creative and copy against those policies before anything goes live, not after an account gets flagged.

Can they show real, documented results?
Ask for actual numbers, not just claims: ad spend, revenue, ROAS, screenshots from the actual ad accounts. If an agency cannot show verifiable proof for other peptide clients, that is worth noticing.

Who will you actually be talking to?
At a lot of agencies, you sign with senior people and then get handed off to whoever is available. Ask who your day-to-day contact will be, and whether that is the same person doing the actual work.

How often will you see real reporting?
Ask what reporting looks like week to week: are you getting a real breakdown of spend, revenue and what is and is not working, or a generic dashboard export once a month?

Is this a partnership or a one-time delivery?
Compliance rules and platform policy shift over time. Ask whether the agency stays engaged to adjust strategy as things change, or whether the relationship ends once the initial build is delivered.
